LINKED-HEARTS is a research program at Johns Hopkins School of Nursing for adults with high blood pressure and diabetes, or chronic kidney disease. 

Eligibility Criteria

  • 18 years of age or older
  • Diagnosed with high blood pressure and at least one of the following: (1) Diabetes or (2) Chronic kidney disease
  • Receive care at Choptank Community Health System, Johns Hopkins Community Physicians (JHCP), or Unity Healthcare

After you join the program

  • Keep seeing your primary care provider
  • Receive up to $200 by completing four health surveys
  • Receive a free blood pressure machine with training
  • May receive counseling on healthy lifestyles and medications for 12 months from a community health worker and pharmacist.
This ​program aims to help people better control their blood pressure. 

Your doctor's office is participating in our research program, and you may benefit from it.
